Aripiprazole
Abilify [BMS}
Tablet 15 mg, 30 mgEig: Artypic antipsychotic with partial agonism on the dopamine (-D2) and serotonin (-5-HT,a) feceptors and antiserotoninergic (-5-HTz) activity. Ari piprazole has a blocking effect. Kinetic data: Absorption: good. F = 87%. Tinax = 3-5 hours. Va = 4.9 l/kg. Plasma protein binding: > 99%. Metabolism: Extensive via the liver, mainly via CYP3A4 and CYP2D6 including to the active metabolite dehydroaripiprazole. Eiminatlie: About 25% via the urine and 60% via the faeces. Less than 1% is excreted unchanged in the urine and approximately 18% is excreted unchanged in the urine.
Z/L: There are insufficient data on the use of this substance during pregnancy in humans and in animal experiments to assess the possible harmfulness. Do not use during pregnancy. It is unknown whether aripiprazole passes into breast milk. Breastfeeding is not recommended.

Ind: Schizophrenia.

Ad: Common (1-10%): Insomnia, restlessness, dizziness, akathisia, somnolence, tremor, blurred vision, nausea, vomiting, dyspepsia, constipation, headache, asthenia. Uncommon (0.1-1%): tachycardia, orthostatic hypotension. In addition, neuroleptic malignant syndrome (formerly known as neuroleptic syndrome), tardive dyskinesia, convulsions, cerebrovascular side effects have been reported in elderly people with dementia, hyperglycaemia and diabetes mellitus.
Very rare (< 0.01%): leucopoenia, neutropenia, thrombocytopenia, allergic reaction, angioedema, pruritus, urticaria, diabetic ketoacidosis, diabetic hyperosmolar coma, nervousness, agitation, speech disorder, QT prolongation, ventricular arrhythmias, sudden unexplained death, cardiac arrest, torsade's de pointes, bradycardia, syncope, hypertension, thromboembolic events, oropharyngeal spasm, laryngospasm, pneumonia aspiration, increased salivary flow, pancreatitis, jaundice, hepatitis, rigidity, myalgia, rhabdomyolysis, urinary incontinence, urinary retention, priapism, chest pain, peripheral oedema, increased creatine phosphokinase, ALT, AST and Y-GT. Survive (attempts) and suicidal ideation have been reported.
Int: The effect of certain antihypertensive drugs may be potentiated. Simultaneous use of antihistamines, antidepressants, barbiturates, sleeping pills, analgesics, opiates or alcohol enhances the sedative effect. During concomitant use of quinidine and possibly other potent inhibitors of CYP2D6 (such as fluoxetine and paroxetine), the dose of aripiprazole should be halved. Halve the dose of aripiprazole during concomitant use of ketoconazole and probably also other potent CYP3A4 inhibitors (itraconazole and HIV protease inhibitors). Double the dose during concomitant use of carbamazepine and probably also of other potent inducers of CYP3A4 (such as rifampicin, rifabutin, phenytoin, phenobarbital, primidone, efavirenz, nevirapine and St. John's wort).
W/V: For signs and symptoms of tardive dyskinesia, reduce dose or discontinue use; the symptoms may temporarily worsen or even reappear after discontinuation of use.
